Endometriosis and Fertility

Endometriosis is a condition in which tissue similar to the endometrium, or the lining of the uterus, grows outside the uterine cavity. This tissue usually grows on the ovaries, fallopian tubes, and other organs in the pelvic cavity, and can cause a range of symptoms and problems, particularly related to fertility. The abnormal tissue leading to inflammation, scar tissue formation (adhesions), and the development of painful ovarian cysts known as endometriomas. These physiological changes can seriously disrupt the normal functioning of the reproductive organs, making it difficult for women with endometriosis to conceive.

The condition is associated with a range of symptoms, including severe menstrual pain, pain during intercourse, chronic pelvic pain and bloating, constipation, and diarrhea. Irregular menstrual cycles and infertility are also common, with endometriosis being a leading cause of infertility in women. The likelihood of infertility increases with the severity of the disease, depending on such as the extent and location of tissue growth, the presence and size of endometriosis..

A comprehensive medical evaluation is crucial for diagnosing endometriosis. This typically involves a pelvic exam, ultrasound, and imaging tests such as MRI. In some cases, a diagnostic laparoscopy, a minimally invasive surgical procedure that allows for direct visualization of the pelvic organs and biopsy of suspicious lesions, may be necessary.  Once endometriosis is diagnosed, treatment is tailored to the severity of the disease, the patient’s symptoms, reproductive goals, and overall health. Early stages are generally managed with less invasive options like hormone therapy, while more advanced cases may require surgical intervention. 

 Laparoscopic endometriosis surgery is a key treatment option for women with endometriosis who are struggling with infertility. This minimally invasive procedure involves making a small incision near the belly button and using a laparoscope—a thin tube with a camera at the end—to precisely locate and remove endometriosis tissue. By removing this abnormal tissue, the surgery helps restore the normal functioning of the reproductive organs, thereby increasing the chances of natural conception. However, for women over 35 or those with additional infertility factors, IVF is often recommended following surgery. This combined approach of laparoscopic surgery followed by IVF can significantly enhance the likelihood of achieving a successful pregnancy for women with endometriosis.
