Uterine Fibroids

Uterine fibroids are benign tumors that form in the muscle tissue of the uterus, and they are a prevalent condition among women of reproductive age. Fibroids can cause symptoms such as heavy menstrual bleeding, prolonged periods, pelvic pain, frequent urination, and fertility issues. The impact of fibroids on fertility depends largely on their size, number and location; for example, fibroids that distort the uterine cavity may prevent conception or increase the risk of miscarriage.

Diagnosis usually begins with a physical exam, followed by an ultrasound to confirm the number, size, and location of the fibroids. An MRI may be used for a more detailed assessment in certain cases. Treatment for fibroids are individualized, considering factors such as the patient's age, reproductive goals, and the specific characteristics of the fibroids, including their size, number, and location. This personalized approach ensures that the chosen treatment aligns with the patient's needs and circumstances.

In cases of severe symptoms or fertility issues, surgical removal of fibroids may be necessary. Myomectomy is a surgical procedure to remove fibroids while preserving the uterus, helping to reduce symptoms and enhance fertility. Depending on the size, location, and number of fibroids, several myomectomy methods may be employed:

- Open (laparotomy) myomectomy:  This method involves a 9-10 cm incision in the abdomen, similar to a cesarean section. A vertical incision may be needed for larger fibroids. 

- Mini-laparotomy: This method involves a smaller incision, usually 3-4 cm, offering a less invasive alternative to traditional laparotomy. This approach allows for effective fibroid removal with shorter recovery times, less pain, and smaller scars, making it suitable for patients who require more direct access than laparoscopy but want a less invasive option. Prof. Dr. Emre Pabuçcu often prefers this approach for its significant advantages.

- Closed (laparoscopic) myomectomy:  This minimally invasive approach requires only a small incision (about 1 cm) at the navel, with 2-3 additional incisions of 3-10 mm for the procedure. Laparoscopic surgery offers benefits like less pain, shorter hospital stays, quicker recovery, and better cosmetic results, but may not be suitable for everyone. The choice of technique is based on the patient's situation and the surgeon's expertise, with laparoscopic surgery generally preferred when appropriate.

- Hysteroscopic myomectomy:  This method removes submucosal fibroids from within the uterus using a special optical device inserted through the cervix, avoiding any abdominal incisions. Hysteroscopic myomectomy is effective for treating intrauterine fibroids and often yields excellent outcomes for both patients and physicians.
